Mission Statement

The mission of Nevada Care Connection is to streamline access to services and ensure that Nevadans receive individualized care that meets their needs.

The vision is that Nevadans with functional limitations and the family members that support them have timely access to correct information and quality services that promote choice, dignity, and independence. 



    Services Offered

    Nevada Care Connection Resource Centers provide one on one assistance to older adults, people with disabilities, caregivers and families.  There are five resource centers throughout Nevada helping people to explore their options, plan for care and connect with the services of their choice. 


    The Resource Centers offer three types of services:

    • Options Counseling - a person centered approach to helping individuals know what services are available in their area.  This is an interactive decision support process that helps Nevadans access the right services at the right time. 
    • Care Transitions - a short-term service that helps individuals improve health and access services in order to reduce hospital admissions. 
    • Caregiver Support - Specialists work with unpaid caregivers (family or friends) to help them explore support options that are available to them.
