FOCIS is a voluntary program that supports Nevadans who are
at risk of nursing home placement or are currently living in an institutional
setting to successfully live in the community of their choosing. FOCIS Health
Care Coordinators are licensed professionals who work alongside recipients,
their support system, and/or discharge planners to determine the recipient’s
care needs. Together a care plan is created to determine which community
resources of the recipient’s choosing will enable them in living in the least
restrictive environment.
Who is Eligible for the FOCIS?
- Nevada residents of all ages who are at risk of
nursing home placement or are currently residing in an institutional setting
who chose community living.
- There are no financial requirements.
How can I apply?
Any individual, their support system, or a provider may call,
fax, or email their regional Aging and Disability Services Division (ADSD) office
to submit a FOCIS Referral.
